Are workers involved in OH&S decisions (consultation)?
The standard requires that workers have a say, not just that they are informed. Whoever stands at the machine often knows the danger better than management.
Are hazards identified systematically?
Without identified hazards there are no effective measures. Auditors check whether rare tasks and maintenance work are covered too.
Is a hierarchy of controls applied?
The hierarchy requires eliminating the hazard first and using personal protective equipment last. Many companies start the wrong way round.
Are workers encouraged to report hazards?
Only when reporting is possible without fear of consequences do you hear about near misses before someone gets hurt.
